when I run my a/c, I find antifreeze coming out of a small drain hose in my rear passenger wheel well. Is this a major concern?

Are you certain it is coolant and not just water? Water normally drains from vehicle after A/C has ran, that is normal.

Ok since you're certain it's coolant than most likely the heater control valve is leaky and needs replaced.
