Gears hard to shift?

Hello there, for the past two days I noticed a serious problem while driving my car. It's really hard to shift the gears, there is no noise or grinding. When I try to shift the gears when I'm stopped, they are all working fine but when I'm driving it's hard to shift.

It sounds like the clutch disc is falling apart, this will cause hard shifting. Please go over this video to help confirm the issue:
