We are getting a noise that sounds like loose tappets comminbg from the top of the engine. Could that be a rod causing problems or would it most likely be that we need to adjust the tappets. 104,000 miles on the van

I don't think your car has mechanical lifters. Most cars today have hydrolic type. Your car probably has hydrolic lifters with are self adjusting. When the top of the motor is making noise (like tappets) I would suspect that there is a oil pressure issue. Is the crank case full or are you down a few quarts of oil. If the oil level is good and you have no other apparent cause for the noise then check the oil pressure. Does the dash have one? If not then you will need to connect and external oil pressure test. I hope this is helpful

I need a little more information like make, model, mileage, engine size and year. Also is the nosie a tapping noise or pinging. This can make a difference indiagnosing the problem and correcting the cause.
