2004 Ford F150 Shaking between 40 and 55mph.

My truck is shaking between 40 and 55 mph. I just had my idler arm and both front hub assemblies replaced and my tires rotated, balanced, and aligned.
this did not fix my problem. What could be causing this shake?

Have you checked the u-joints? When the tires were rotated, was there any change?
