2001 Ford Escort slipping

I have a 2001 Ford Escort, when I drive my car about a mile or so it would drive fine until I have a stop. As I step on the gas pedal in the start of the go my car will jurk into the next gear. It doesn't do this all the time just when it is a hot day or I go on the freway and get off then go again. What if anyting can I do to fix this problem?

Yes my transmission was flushed. It would start to jerk after I run the air conditioner to.

Are you sure the trans is the issue and not an engine miss?
