How do you get at the brake light for replacement? I do not see any visible fasteners or screws on the exterior, and there doesn't appear to be any access points from behind.

Open the back hatch. To the side of the light housing where the hatch would normally cover it you will see what apear to be two balck round screw covers. These are a type of snap pin retainer. Eash one is made of two parts. Gently pry the top part up with a thin blade screwdriver. Make sure to pry it out evenly, not all from one side. It will release. When you get both removed for the light housing will be loose. Don't let it fall out of the body. When reinstalling the light housing be sure that the guide pin lines up properly. Then reinstall the two retainers pushing the top part down as it was before. I think you will find it easier than it sounds. Charlie T
Also, if replacing the center high brake light, leave the lift gate closed and remove the two screws (#25 torx usually). Then carefully pry the lens module out being careful not to damage the gasket. Now pop out the bulb and install the new one. Reverse this procedure to reinstall.
