2000 Dodge Caravan transmission shiftdown

When my van gets warmed up it shifts from drive down to third and won't go back in to drive. At 100km/h, suddenly the tach zoomed up to 5000 rpm. Anyone encounter this problem and do you have any suggestions?

solenoid pack on the side of the tranny is what common
