At cold temperature van runs fine. As car or air temp warms, the van starts to 'buck' and hesitate when stepping on the gas. I need to accelerate very slowly to keep from stalling. When the car cools down, it runs fine again. What could this be?

Your going to need to test for fault codes with a scanner, a bad ECT will not tell the PCM to back off on the fuel needed for changes in temperature. The plugs are going to need cleaning or replacement after you see what codes you have. The engine coolant temperature sensor is one of the important sensors.
Hope it helps, ok It's usually near the thermostat area.
