1991 Dodge Caravan Rough running

I have a 1991 Dodge Caravan that runs fine at idle and when I have it WOT. When cruising at around 2000 to 2500 rpm it acts like it is starving for fuel and is going to die. The check engine light does not come on but it gave me the codes 12, 21, and 44. Is this a fuel filter problem, fuel pump, or maybe a tps going bad?

#12 battery conection, battery has been disconected within last 50 key on cycles
#21 oxegen sensor signal circuit. Sensor voltage not fluctuating
#44 battery temp sensor volts malfunction, problam with battery temp voltage circuit in the pcm
