On cold mornings, I raise the garage door about 2 feet, and start my van. Is that enough ventilation?

That is where the fuel vapors go is on the ground, so the 2 feet should be amply. If you can buy a hose that goes over the exhaust pipe and vents it outside. With the emissions these days on vehicles you do not have to worry about intoxicating your self or gasing yourself out the worse you can get is a headache.
