Sounds like it may be a sensor problem. Check and or replace the engine coolant temperature sensor. This sensor tells the computer that the engine is hot or cold, by a sliding voltage scale, your sensor is probably stuck at the high end, constantly telling the computer that the car is hot, thus keeping the fan on
