1999 Chevy Truck Transmission jerk

whenever I go about ten miles or more on the highway when I get off at the exit and completely stop the truck, whenever I start going again the transmission jerks really hard when it shifts. Feels like the torque converter is locking up or like popping a clutch. It only does it when I get off of the highway. I can drive around town all day and it never happens. When I'm getting off the highway if I get the truck going about 15-20mph and hit the gas again so that it downshifts it usually won't do it. Or if I can just roll through the stop light at the exit. Also I can manually downshift to 2nd or 1st gearwhile stopping and then put it back into drive and it does fine.

A good spot to start is to see if any codes are in the computer.

After checking this, post them and include the liter size of the motor and the model of the truck.
