1998 Chevy Truck surges

I have a 1998 chevy pickup and it started to stumble and hessitate if you hold it steady. It will surge going down the road. I have replaced the fuel filter but did not fix it. I cleaned the IAC valve and checked vaccum lines for leakes but no change. Thanks for the help.

Sounds like a sticking exaust valve or a failing head gasket.

i think I got it figured it out. I suspected bad gas and I poured some b12 in it and immeditely seen a difference but still some hesitation. I am gonna try to flush the fuel system and hopefully that will fix it for good. Thanks everyone for the help.
