1999 Chevy Suburban

I have a 1999 Chevy Suburban 2wd with custom 18" rims. When I'm around 40mph the steering wheel starts to shake excessively especially when I apply the brakes real hard. What could be the problem?

Possibly the front rotors are warped. Probably turned during the last brake job, but without adequate thickness left.
