Hi there,

With out being able to investigate the failure first hand I can only offer my summation on what may off happened, there could have been a number of reasons, but I feel the most likely are a failure of the tensioner, a chain failure due to stretch, this could also account for a tensioner failure as if the chain was well over length due to wear the tensioner may not have been able to take up all the slack, a failure of a chain runner allowing to much slop in the chain, a failed tooth on a drive gear, maybe a closer investigation of the failed components will shed some light on this for you.
