Rocker arm stud nuts

I have a 1998 blazer 2wd 4.3L V6. After removing the heads and replacing a blown head gasket, I can't get the rocker arm nuts to stay tight. I replaced all of the nuts (12) with new ones. I took out the lash and turned each nut 3/4 of a turn as the book stated. Do you adjust it while running?
The Haynes repair book gave two adjusting points, on TDC for #1 and TDC for #4. I followed instructions but some nuts are still coming loose.

I am not 100% on the adjustments on those trucks, Ive got experience with cyl hd work, Ive got a question to ask, On the top of the stud were you hd nut is placed, On the stud is there little slot on top. For example that a flat head screw driver could fit. If so then Once you use a feeler gauge measuring tool, then with the screw driver in the little slot on the head bolt, tighten the nut, Things should work out for you.
