1995 Chevy Blazer stalling

After the engine warms up (say 15 mins) the engine starts to idle rough and stalls. Wait 1 min. And it restarts fine for another 10 mins or so and it happens again. If I rev it up to about 6000 rpms for a few seconds seems to clear it up. For alittle while.
Is there a sensor or valve that I can replace to solve this?

Cross these two terminals with a small piece of wire, or paper clip as shown, then turn the key on. Count the flashes, a 22 will flash as 2 flashes then a short pause, then 2 flashes, if more than one code is stored, the pause will be longer. The codes will display three times and end with 12.
