2002 Chevy Avalanche Rough startup in cold weather

My 2002 Avalanche with 113,000 miles on it has a problem with a very rough start up during cold weather (below 40 degrees). It bucks and jumps and surges and misses for about 5-7 minutes. Once it is warmed up it seems to run fine. The engine light does not come on. For these vehicles and engines I have read that it could be a manifold leak, a faulty gas pump, an air throttle valve problem or using 10% ethanol fuel. I have yet to take it to a repair shop. I am curious as to what the real problem might be since this problem seems to be fairly common.

Clean out the idle air control valve, test the following coolant temperature, throttle position, and manifold absolute pressure sensors-to include the PCV and EGR valves
