2005 Buick Rendezvous can't accelerate past 15-25 mph

Engine Performance problem
2005 Buick Rendezvous 6 cyl All Wheel Drive Manual 80k miles

Service Engine Soon led illuminated. Car goes into limp mode; can't accelerate. It seemed as if the fuel flow was blocked. I added fuel injector cleaner to gas, problem went away. However, the alert is on again.
