What would be the indication that I need to replace the crankshaft balancer on my 3.8L 91 Buick Regal?
I am hearing a noise from the area of the balancer even with the drive belt removed. It can be heard from the passenger wheel well with the hood closed. It is most noticible in winter cold weather.

It could be any of the pulleys! Harmonic balancer will wobble. Waterpump will have a tinny clanking sound, power steering will whine. Tensioner or idler pulleys will screach and so will a/c and alternators. Although with the belt off they won't, but a waterpump driven by the timing belt will still make noise.
