A/C Blower Motor fan speed is either Super slow or very high?

The A/C in my truck gets cold and can fluctuate between temperatures as needed very well. However, it seems the fan speed is either SUPER slow (almost nothing coming out of the vents), or very high if set on the highest setting. If I set the speed to anything within the middle, it stays at a fairly low speed until I turn it to max speed. I will say even the "Max" speed does not seem as strong as it used to be.

I pulled the blower motor out and cleaned it. I have also cleaned the cabin air filter. Does this sound like the blower motor could be starting to go, or maybe the control module needs replaced? To be honest I'm not sure if these 2014 Sierra's had a separate control module?

Yes, the blower motor control module is part of the blower motor here is how to change the unit out in the images below which should fix the problem. Check out the images (below). Please let us know if you need anything else to get the problem fixed.
