The inside boot on the driver's side CV joint has separated, so I can see inside the joint. I get a rumbling noise when I drive. Does the boot just keep dirt out, so that I could replace it and feel confident, or does it contain grease, so that some mechanical wear might have occured and I might need to have the joint replaced?

Both. The boot is shaped like an accordion to push grease back into the joint as it rotates. The boot can be replaced, but if the grease has sprayed out and dirt has gotten in to the point of making noise, you are better off with a remanufactured half shaft. The cost has come down a real lot in the last ten years to where the half shaft costs less than a new joint.
