Air is only blowing through front vent, when selecting defrost or floor, there are no changes in air flow, front vent remains active. Temperature is fine. Can someone please advise which actuator would need to be replace?

Yep, this problem will be the mode blend door actuator. This actuator is above the accelerator pedal on the left side of the dash so it might be difficult to get to and they say to you need to remove the steering column and the speed control deactivator switch electrical connector.

Do not press the brake pedal when removing or installing the speed control deactivator switch, as the switch will be damaged.

To reinitialize and calibrate the actuator stop points. To carry out calibration, carry out the following steps.

Remove Smart Junction Box (SJB) fuse 15 for at least one minute.

NOTE: When the ignition switch is switched to the ON position, the HVAC module will initialize and calibrate the actuators. Calibration of the actuators will take approximately 30 seconds.

Reinstall SJB fuse 15. Turn the ignition switch to the ON position and wait 30 seconds before verifying correct mode door operation.

Here is the exact location and the mode blend door actuator. Check out the images (below). Let us know how it goes
