1991 Acura Integra

I have a 1991 integra gs with a 1.8 liter dohc with about 164000 miles and a 5spd standard transmission when I first start taking off the acceleration struggles just a little bit but barely noticable as I get higher and higher into the rpms this whining noise gets louder and louder and my gas mileage has gone down and the car has gotten a little shaky I dont no if its my cat or some kind of fuel problem?
