My car is making a clanking soung (like nails in a tin can) when I start it up. It also revs up very high before finally shifting into the last gear when accellerating past 60 mph. Could my transmission be going out?

I also recently had the transmission fluid flushed during a routine oil change and was told that was a BAD thing to do.

During start up a noise such as the pitch you desicribed could be a simply an exhaust head sheild and as the engine is torqued during start up creates it to wrattle. Sometimes it will make noise and other times it will not based on how the exhaust vibrates.

The reason I say this is because transmissions will not usuaully make this noise unless catistrophic failure occured leaving you immobile.

In term of the transmission shifting I would have the transmission fluid level checked while warm. What fluid was used?
