What would cause the upper Bolt to break on the cerp belt tensioner?

I have a 05 odyssey with 98000+ miles and just had the water pump and timming belt replaced. Also early this spring I had the power steering pump replaced for the 2nd time and cerp belt replaced. Since I had the timming belt/water pump replaced about a week later the upper bolt broke on the cerp belt tensioner. My Mechanic replaced the bolt and it only lasted approx. 1 week. What's causing this? Is the tensioner or did they not put in the proper bolt? Thanks for any imput.

It cold be either of the things you mentioned. It also depends on wher ethe bolt broke, if it broke in the block or in the middle, etc. There is a tsb that the timing cover needs to b modified for later tensioners as the design has changed. I'll send the pic along.
