Transmission not shifting from third

All the dash lights are on they keep flickering and it shifts from 1-3 running 4,000 RPM's at 40 miles in hour.

This is kind of a complex issue to some up in one sentence. Could you give me a detailed description of what is going on? I don't quite follow with what you wrote.
