The standard pcv valve has one vacuum hose running to it and you pull it straight up out of the valve cover. (The elbow frequently cracks on this type so be prepared to find another one or tape up the crack until you can get a new one.)

They also made a heated pcv valve. This one has three lines running to it. Two of the lines are connected to the water pump, so they have coolant running through them [this heats the pcv valve]. The large line on top is the vacuum hose to the pvc valve. You'll have to remove both water hoses and the vacuum hose, pull the pcv valve "assembly" out. The assembly is held together by a clip. Remove the clip, which now allows you to remove the pcv valve. Put the new pcv valv in place [it takes a standard pcv valve; by the way], install the clip. Put pcv valve back into the valve cover, connect hoses and you're done.
