I have a 93 Jimmy with a 4.3 vortec that is doing the same thing. I cleaned the EGR valve, OCV valve, and the idle air intake control valve and now it rarely shifts into overdrive. Runs a lot better and has more power but it rarely shifts all the way out. How did you fix it?
