Everytime I push on the break it jerks really bad. Why is that? What could it be

That can mean a lot of things. If you feel a pulsation in the brake pedal the rotors are likely warped. They can be replaced or they can be machined if they will end up above the published legal minimum thickness. Machining rotors is a normal part of a brake job.
