1994 Ford Truck exhaust valves

Twice vie' had an exhaust valve burnt in a pie shape fashion on this engine. I was wondering if it was caused by too much back pressure as in a stoped up catalitic coverter or not not enough back pressure as it running a strait pipe ex hast, or is more lickly that itis caused by runnig high test, (high octalin)) fuel?

It could be caused by a few things, a vacuum leak making cylinder lean, valve not adjusted correctly, timing off or a bad injector. All of that would need to be checked first.
