When I take off hard in 1st or second gear after like 4k Roma it starts to jerk really bad

Always start with the basics on a car that runs well for the most part. Id check/change the plugs and wires and the air filter and fuel filter first. Theyre often forgotten and it cant hurt to change them if you cant remember having done it.
