1991 Isuzu Trooper Startup

In the morning when I cold start my car runs at 400 rpm until war. When I start it and its warm it stumbles and dies unless I give it some gas then it runs fine. I've recently done a tune up new coil new egr and cleaned the idle air control valve and throttle body.

It sounds like you may have a fuel problem. I would test for proper fuel pressure at the fuel rail. From what I can gather on your vehicle, it should be between 9 to 13 psi. Im not 100% on this specification since my information on this vehicle is very limited. I would also check the fuel filter to see if there is any contamination and replace if it looks clogged. If this has never been replaced, I would replace it anyways due to it being a maintenance item. Lets start here for now.

I replaced the fuel filter yesterday and no change I was thinking fuel pressure regulator maybe?

I would still test the fuel pressure to see what its at. It could be the fuel pump as well not producing enough pressure or it could be what you stated, the fuel pressure regulator. I would also inspect the fuel injectors for any contamination and make sure there isn't any fuel getting by the injector seals.
