88 Sundance problem

I have a 1988 Plymouth Sundance with the 2.2 TBI engine. To start here are the problems.
Stumbles over 4000 rpms
Idles at 1500 rpms (which I think is twice what it should be)
Occasionally the idle will jump to 4000 rpms and die at stop signs

I recently changed the plugs, wires, cap, rotor, and air filter which fixed it for a couple of days. I will be changing the fuel filter on Wednesday. I have also checked for error codes (there were none).

Could this be a vacuum leak?
Is there anything else I should check?

More issues now.
Car is now throwing code 13 (Map sensor)
Replaced all vacuum lines and map sensor which fixed the issue until the car warms up. Once it is completely warmed up it throws the code, and begins idling erratic bouncing between 800 and 1200 rpms.

If anyone can help it would be greatly appreciated.
