Clutch Problems 1996 Nissan 200SX

I have a 1996 Nissan 200SX 1.6Ltr, 155,000 miles.

Is there a way to adjust the clutch? I had a new clutch put in about 4 months ago. Now, the clutch pedal is all the way to the top. I just barely touch the clutch pedal and the clutch enages. Also, if I attempt to start from a stop with any type of quickness the clutch slips (spins).

When the tach goes crazy is the engine reving during this as well to confirm that the tach is accurately monitoring the engine? Are you getting a check engine light on the dash?
