The Ranger runs fine forward but when cold the truck will not back up. Once the truck is warm, it backs up fine. I checked the transmission fluid and it it full. I had a mechanic check it and he said the pressure was a little low. Does this mean there's no alternative but to replace or rebuild?

Have you done a trans service yet? If not I would do a service. This is where you remove the trans pan and replace the filter inside and fill back up with new fluid. The proper fluid is dextron/mercon.
