Check engine light comes on

Recently my check engine light came on. Using my meter light code I found meter light code 21 on scan tool code P0140. Since the car had 119,000 miles and I decided to replace all of the o2 sensors(four of them) I then cleared the code using the scan tool. The check engine light stays off, until I drive 50 miles or further. The scan tool then says "Down stream 025 stays at center" Can you help me on how to fix this problem or what to do? Thank you

What is the code it is displaying now, to make sure it is accurate, clear codes and re drive until light re sets and re scan, post the code set.

A for things to start checking are, shorted heater circuit in O2 sensor. Shorted signal circuit in O2 sensor. Melting of harness connector or wiring due to contact with exhaust system Water intrusion in harness connector or PCM connector Bad PCM, have a look into these first.
