Battery is five years old, after recharging, it started up fine but would not restart after shutting it off?

Had to recharge my battery today to get it started. It's 5 years old, so I'm not surprised. After recharging, It started up fine but would not re-start after shutting it off when refueling. I am getting a P1000-00 "OBD systems readiness test not complete." On my Innova 5410 scanner. Prior to this, a couple of days ago, I did have a "P0690 ECM/PCM Power Relay sense circuit high" code. I did totally disconnect my battery to clean the terminals a day ago, but it started after that. Okay, I found that error code and it looks minor/normal for having disconnected the battery. Still, the engine will not start. Looks to be intermittent problem. I know, the worst kind huh? Again, thanks for your thoughts. Just sitting here scratching my head.
Thoughts?

The codes are there because of low voltage, I would replace the battery, is sounds bad but to be sure this guide can help you load test the battery using no equipment:
