Why does my car start to jerk when i drive it.

It does it while im driving no matter if im driving in town or on the highway or even the interstate, it mostly does it when i go up hill but it will also do it while im driving on a flat straight road. it also does it when i accelerate to, it doesnt consistantly do it but it will do it a couple of times while im driving, i changed all the fluids but it still does it.

Can any one help me with this?

Do you have a check engine light and when was last tune-up?

Will it shake when its idling

The check engine light is on and the last tune up was 3 months ago.

It doesnt shake while idling.

CEL On-scan the computer for code/s and get back if any with the exact code number not description of problem
