Where exactly on the male fitting of the high pressure fuel line to the fuel injector rail does the O ring need to be placed. Does it go between the two ridges or all the way against the flange? When I tried to put the factory purchased O ring in between the ridges it still leaked. I took it apart again and the O ring was in two pieces, could it have dislodged when I tightened it up? Does the O ring go between those two ridges?

If the o-ring you're talking about is roughly the diameter of a pencil, then yes, it does go between the ridges.

To prevent a leak there wrap the thread of the fuel line with teflon tape (or other pipe sealer). Hold the line in its proper position while tightening the line by hand, continue to hold it there while tightehning it with the wrench.
