Transmission fluid

I have a 2000 toyota 4-runner, 4wd with 122,000 miles. I just had it in for its regular oil change and was told that I had tramsmission fluid in the engine coolant. It just had the plugs and wires replaced along with rear brake shoes 2 months ago. Prior to that it had a cooling system flush and new thermostat in Jan, 07. And had transmission flush 1 year ago along with its regular oil changes. What could have caused this?

First off look for yourself.

The internal cooler for the transmission fluid may have ruptured into the coolant section of the radiator. IF so, you need to either replace the radiator, or install an external transmission cooler and plug off the ports at the radiator for the transmission lines.
