Hesitates on soft acceleration

soft acceleration hesitation.
hard acceleration OK, idling OK, starting OK

Could it be:
bad gas? Vacuum leak? Sticking linkages? Plugged fuel ports? Clogged air filter or other problem with air intake? Bad fuel pump? Bad fuel filter? Electronics? Spark plugs?

When was it last tune-up

Could be a bad TPS/MAF/dirty injectors/vacuum leak/low fuel pressure. Hook-up a scan tool and view the parameters data stream what exactly are they doing.
