Car acting jerky

When I am traveling on the interstate going 80 mph and I have to go up a hill the car shifts into high gear and then it jerks, it is a two step process every time. Then when I get to my destination and stop at a red light and when the light turns green and I press the gas to go again it jerks. It only occurs at high speeds and then after long trips at high speeds at the first full stop. What is this?

Have the transmission checked, there may be a clogged line or the filter may need to be changed. There is also a down shift rod. Check so it has not slipped
