1995 Honda Passport what is the problen?

I have manual 5 speed. Out 5 gears only # 4 works
please if you can tell what it may be the problem?

I take it the clutch works ok? Pedal feels normal? No reverse either? Could be a the trans has broken internally. Does the shifter feel normal as far as the bushings?
