1994 Honda Civic Timing Bet Replacement

I need to replace the timing belt and maybe the waterpump on my son's '94 Civic DX. He's got over 120k on it and I'm trying to be preventative with the timing belt. I will do the water pump as well if it's not that much more trouble.

I've got the haynes manual and I planned on doing it per the book; but I've got a problem. The diagram in the haynes manual shows the numbered cylinders and distributor cap positions different than the way it is on his car.
It shows the first cylinder on the right is No. 1 just like I thought but it shows number one on the cap to be directly opposite where his car is currently connected.

How is that possible? is his timing 180 out? should I change it when I do the timing belt? Do I need to take all the sprockets and everything loose to put on the new belt or only if I'm doing the water pump as well? Any advice is helpful

Thanks, those pictures help; but I'm trying to get a sense of how much I have to take loose to do the job. I have the haynes manual and it's pretty clear (except for that whole TDC business) but do I have do totally take everything apart to do it. Can I just loosen a pulley or sprocket, slip the old one off, and slide the new one on. Is it a good idea to do the water pump at the same time?

As you see in the pictures
loosen tensioner and remove the belt
I would also replace water pump too

you have to set timing and align the gears
you can not slip one out and new one in
all should line up with marks as the book shows you
