When attempting to shift the transmission into fourth gear there is a noise?

Before the client knew me, he has been battling with his car listed above Touring which had gear issues. Two have been fixed by different technicians but one still remains unfixed. The first one, according to the client, was water that was mixed with the transmission fluid. Thus, a complete transmission was replaced.
Another was transmission fluid leaking slightly which was also fixed. Unfortunately, one still remains unfixed. Whenever you on motion the gear changes from 1-3 it selects smoothly. But when the gear is about to go to 4 gear, it's accompanied with a noise.
Yesterday, I retrieved all system scan codes. They're the first three screenshot


When I cleared the all system scan this is what I retrieved from the last image.

Please how do I fix the car problem?

Did they replace the radiator as well? If not, there is coolant inside the transmission again. Also, with all of these codes I would clear them and then restart the engine for a minute and then run the codes again. You may have a bad PCM. Please get back to me after you have done this so we can continue. Can you please shoot a quick video with your phone so I can hear the noise? That way I can tell for sure what's going on. You can upload it here with your response.

I'm so sorry for responding very late to this message! I was so preoccupied with many activities that hindered me momentarily from working.

Thanks for the recording. There is something wrong inside the transmission like a planetary gear set has gone bad. Sorry, but the transmission will need to come out.
