At cold start up it keeps dieing, unless I keep my foot on the gas. Once moving, in feels like it is jerking back & forth and doesnt really want to go until I push down on the gas pedal harder, once I get up to about 60 mph is seems to run ok, whenever I push on the gas pedal a little harder it seems to want to go ok, but trying to stay at 30 mph(for example) it just starts jerking like its not running on all cylinders.

Oxygen sensor
Catalytic converter
Dirty fuel injectors (cleaning the injectors often fixes this).
Bad MAP (manifold absolute pressure) sensor
Bad TPS (throttle position) sensor
Bad or dirty MAF (mass airflow) sensor
Low fuel pressure (leaky fuel pressure regulator or weak fuel pump)
Vacuum leaks (intake manifold, vacuum hoses, throttle body, EGR valve)
Bad gasoline (fuel contaminated with water or too much alcohol)

Sometimes, what feels like a hesitation is actually ignition misfire rather than lean misfire. The causes of ignition misfire may include:

Dirty or worn spark plugs
Bad plug wires
Weak ignition coil
Wet plug wires
