Transmission Troubles

My problem seems that the trans upshifts to soon, at low speeds. It's into 2nd by 15 - 20 mph. Then it wants to jump in and out of 2nd to 3rd and back. It also (sometimes) won't go into 3rd on the highway when it's cold. It also downshifts into 1st hard when stopping. I tried the Throttle Valve cable adjustment steps and it seems to work, but then reverted back to the same symtoms as before. The TV adjustment said to depress the D shaped button and move the cable to the stop, but didn't say to move it up or down. I pulled it up and the cable covering came out of the holder. I put it back and did the WOT and heard 1 click. I am not sure if I am doing the adjustment proper.
