2010 Ford Explorer error code P0191

I faced a problem that my Explorer RPM fluctuating and the car stop moving while the engine still running I checked with my ACTRON code reader and it showed code P0191 (Fuel Rail Pressure sensor A circuit Range/Performance)
Where could be the problem? Is it the sensor? Or it might be the fuel pump?

check fuel pressure with a gauge first auto parts rent it and clean throttle plate on both sides an diac hole with choke cleaner. shold be 30-40 psi key on engine off

The fuel pressure is OK.
But I found that when I leave tha car running for long time fuel pump start giving abnormal sound.
I changed the fuel pump and filter, now my car performing greatly.
