2004 Isuzu NQR Noise

I have a 2004 Isuzu nqr. It runs fine on flat land but when going up hill, it starts to make a loud shuttering noise. It sounds like an old manual transmission when its not quite in gear and you let the clutch out. Like if a clutch on a manual trans. Was put in backwards and the sound of the springs rubbing up against the crankshaft bolts. I don't want to think I need a new trans. I am hoping it is something simple. Does anyone know what it could be? Help! I have a job starting really soon up in the Hollywood hills and I can't afford to have my truck be out of commission. Thanks, Mike

Is this the box van? Have your drive shaft/uni joints/carrier bearings checked out.

The chassy has a utility box mounted on it. The drive shaft, hand break, trans. Tail shaft all checked out perfectly. It only does when I start going up a hill. HORRIBLE NOISE! I immediately left off the gas. Not sure what would happen if I just kept excellerating. Sounds almost like a manual trans. Slipping out of gear.
